If you would like to fill out our Easy Form, Follow these steps:

1. Download and Print the forms you want here:

2. Fill out the Forms and Scan them back into your computer.
3. Fill out the personal and company information to the left.
4. Check the “Yes” box for the question “Are You Uploading an Easy Form?”
5. Upload the scanned form in the “File Upload Field” and click submit.
